This WAIVER and RELEASE is executed on (date)(print) ________/_____/_______ by (requestor, PLEASE PRINT NAME) ____________________________________whose address is _______________________________________________________________ in favor of Crooked Creek Conservation and Gun Club, Inc. whose address is 13203 East 246th Street, Noblesville, IN 46060.
Whereas requestor acknowledges that (i) he or she will be participating in fishing, hunting, target shooting, dog training, (whether by foot, or with the aid of a motorized conveyance, or by horseback), or other outdoor recreational activities on Crooked Creek Conservation and Gun Club Inc.’s property, and (ii) he or she is willing to WAIVE and to RELEASE Crooked Creek Conservation and Gun Club, Inc. from any and all claims, demands, caused or action, liabilities, losses, damages and costs that may arise from any and all injuries and damages sustained by him or her while participating in fishing, hunting, target shooting, dog training (whether by foot, or with the aid of a motorized conveyance, or by horseback), or any other recreational activities for himself, or herself, his or her spouse, his or her guest(s), and his or her heirs, executors, administrators, personal representatives and assigns, hereby AGREES that Crooked Creek Conservation and Gun Club, Inc. and its’ officers, directors, agents, employees and affiliates shall have no liability whatsoever to the Requestor for any injury or damages to the Requestor or to any property of the Requestor sustained by him or her while participating in fishing, hunting, target shooting, dog training (whether by foot, or with the aid of a motorized conveyance, or by horseback), or other outdoor recreational activities and hereby RELEASES Crooked Creek Conservation and Gun Club, Inc., its’ officers, directors, agents, employees and affiliates from any and all claims, demands, causes of actions, liabilities, losses damages and costs that may arise from any and all injuries and damages sustained by him or her while participating in fishing, hunting, target shooting, dog training (whether by foot, or with the aid of a motorized conveyance, or by horseback), or other outdoor recreational activities, whether caused by Crooked Creek Conservation Club, Inc.’s negligence or otherwise.
The Requestor has read, understands, and voluntarily executes this WAIVER and RELEASE OF LIABILITY, and further agrees that no oral representations, statements, or inducements apart from the foregoing written agreement have been made.
